Job Title: Case Coordinator
Major Bronx home care services company is seeking a dynamic Case Coordinator to provide general direction and supervision to Personal Care Aides and Home Health Aides providing home care services consistent with applicable regulations and agency policies. Responsibilities include training; planning, assigning, directing work, monitoring attendance; evaluations; addressing complaints and resolving problems. Maintain and monitor client service schedules on a daily basis and to create, modify and manage schedules of PCAs and HHAs. Responsible for determining and verifying the number of hours the PCAs and HHAs worked. Determine the need for involvement of other members of the client’s health team (physician, clinician, social worker, etc.) regarding the plan of care and consults with supervisors and staff, individually or in groups, regarding clients or families, special programs, or training programs. Reach out to HCSP, CASA, PSA, ACS and related community service agencies regarding client home care services
Experience and/or Skills Required
- A minimum of two (2) to five (5) years of experience working in a health care setting such as a homecare agency with at least two (1) years of supervisory responsibility (preferred)
- Ability to coordinate responsibilities. Excellent organizational and time management skills required.
- Working knowledge of computer software and electronic health record systems.
- Demonstrated competency in written, verbal and computational skills to present and document records in accordance with program standards.
- Demonstrated competency in development and implementation of the interdisciplinary planning process.
- Excellent interpersonal skills required.
- Bilingual

Pay: $45,000.00 - $46,000.00 per year
