Job Summary: As a Case Manager, you'll provide targeted mental health case management services to clients to help them gain access to medical, social, educational, vocational, financial, and other necessary services as they relate to the recipient’s mental health needs.
Supervisory Responsibilities: Coordinate services with fellow Fernbrook employees but not responsibly for their supervision.

Position Type and Expected Hours of Work
- Flexible hours between 7:00am – 7:00pm.
- Travel to various locations required.
- Salary staff expected to work 40-43 hours per week.
- Hourly staff up to 40 hours per week, with overtime approval required.
Duties and Responsibilities
- Utilize effective strategies to respond to client needs.
- Maintain accurate documentation.
- Coordinate referrals, discharge planning, and collaborate with community support services.
- Meet with the client at least once every 30 days or as requested.
- Coordinate and facilitate quarterly team meetings.
- Advocate for resolve crises and reduce barriers.
- Seek out supervisory as needed.
- Participate in staff meetings and clinical supervision sessions.
- Complete continuing education requirements.
- Uphold Fernbrook's mission and policies.
- Meet productivity standards for both positions.
- Perform other assigned duties.

Education and Experience
- A Bachelor’s degree in one of the behavioral sciences or related fields, including but not limited to social work, psychology, or nursing from an accredited college or university.
- Pass criminal background check.
- Reliable transportation and valid driver's license for community-based roles
Preferred Education and Experience
- Experience with diverse client population.
- Bilingual proficiency.
