What You’ll Do
- Provide mental health case management services to adults along side county workers
- Assist clients in accessing medical, social, educational, vocational, and financial services
- Coordinate referrals and collaborate with community support services
- Advocate for clients, helping resolve crises and reduce barriers
- Maintain detailed and accurate documentation according to quality standards
- Participate in ongoing training with 30 hours of continuing education every two years
- Engage in team meetings, program development, and client consultations
What We’re Looking For
- Ethical decision-making skills and strong professional boundaries
- Ability to independently manage schedules and deadlines
- Strong communication skills (written & verbal) with internal and external staff
- Ability to work in various settings independently and as part of a team
- Willingness to seek feedback, learn, and grow in clinical knowledge
- Proficiency in computer use and basic software programs (or willingness to learn)

Education & Experience
- Required:
Bachelor’s degree in behavioral sciences or related fields (social work, psychology, nursing, etc.), OR - Three to four years of experience as a Case Manager Associate, OR
- Registered Nurse with psychiatric training and 3 years of community mental health experience
- Preferred:
- Experience working with children, adults, and families
- Bilingual (a plus!)
- Prior experience as a Case Manager
- Knowledge of local community resources

Work Expectations
- Work primarily Monday–Friday, 8 AM – 5 PM, with some flexibility based on client needs
- Travel to client homes, community settings, and Fernbrook offices
- Must have reliable transportation, a valid driver's license, and insurance
