JOB SUMMARY

In collaboration with the Chief Nursing Officer and the Nurse Manager, the Residential Care Coordinator is responsible for planning, implementing and over-seeing the care provided to Non-Skilled Swing Bed (NSS) residents.  The Resident Care Coordinator (RCC) will be support the ongoing evaluation of the resident’s comprehensive plan of care. Uses established work systems and processes to facilitate quality of care and living environment, while ensuring ongoing regulatory compliance with all applicable laws and professional standards of practice.  The RCC assumes nursing leadership functions by establishing workflow changes and performance improvement initiatives in the NSS unit.  Facilitates effective communication of patient needs and issues to the interdisciplinary health team and fosters positive change to maintain a collaborative team environment.

 

ESSENTIAL DUTIES AND RESPONSIBILITES

 

Care Management:

  • Serves as the primary communicator and support contact for the NSS Provider and family members.
  • Completes the Resident assessment, intake documentation, care plans and other assignments to meet Federal and State regulations and quality expectation. 
  • Collaborates with CNO RN to conduct regular care plan review meetings.
  • Works on the unit as an NSS nurse.
  • Ensures appropriate staff are informed of all changes in condition and updated care planning needs.
  • Manages scheduling, completion, and documentation of monthly Provider rounds.
  • Manages scheduling, completion, and documentation of patient and family care plan meetings as indicated.
  • Collaboratively works with residents, families, nursing staff, and Attending Provider in managing the care processes for each resident, ensuring that plan of care is assessed, reassessed and altered to meet evolving care needs of each resident, and each resident’s goals are clearly identified and that appropriate interventions and timelines are established and communicated to the team.


Leadership:

  •  Works collaboratively with Nurse Manager and CNO in guiding nursing practice in the NSS unit.
  • Supports therapeutic care and projects with the Activities Coordinator.
  • Maintains communication within the health care team, other hospital departments and with outside agencies as needed.
  • Identifies unit program goals and objectives, collaboratively develops and implements action plans with members of the healthcare team.
  • Provides leadership and assistance to the Resident Council to address concerns or needs. Ensures council conducts regular meetings with documentation of minutes.
  • Actively participates in the Hospital Staffing Committee and Medication Administration and Safety Team (MAST).
  • Reviews safety events submitted within the NSS unit and collaborates for follow up with other interdisciplinary staff members when necessary.
  • Performs other related duties as assigned by management.


Manpower Management:

  • Participates in the interview and selection process of NSS nurse applicants.
  • Provides performance feedback to the NAC Supervisor and Nurse Manager when LPNs, RNs, and NACs work in the NSS unit. 


Patient Care Delivery:

  • Ensures that staff are informed of changes in the clinical condition of residents, alterations in the plan of care and implementation of best practices to ensure patient and staff safety.
  • Collaborates with Informaticist to adjust electronic health record documentation workflow processes to maintain regulatory compliance and optimize efficiency.
  • Identifies gaps in care delivery processes and collaboratively participates with members of the care team, providers and nursing leadership to resolve.
  • Reports equipment needs for the unit during annual budget preparation. Identifies and reports needs for NSS unit repairs to maintenance personnel in order to assure environment of care is safe, in good repair, and inviting for residents and families.

 

PROFESSIONAL REQUIREMENTS

  • Actively checks and responds to emails throughout the day.
  • Completes all mandatory education within given timeframe.
  • Maintains regulatory requirements.
  • Reports to work on time and as scheduled.
  • Wears identification while on duty;
  • Represents the organization in a positive and professional manner in the community.
  • Attends annual review and department in-services as scheduled.
  • Attends at least 75% of staff meetings; reviews all monthly staff meeting minutes.
  • Actively participates in the department’s quality improvement (QI) activities.
  • Complies with all organizational policies regarding ethical business practices and the employee handbook.
  • Communicates the mission, vision and values of the organization as well as the focus of the department.
  • Meets dress code standards for the department; appearance is neat and clean.
  • All Ferry County Health employees are required to maintain confidentiality in accordance with organizational policy, state and federal regulations, including not but limited to, the Health Insurance Portability and Accountability Act (HIPAA).

 

JOB KNOWLEDGE & QUALIFICATIONS

 

Education and/or Work Experience Requirements


  • Current LPN or RN license in Washington State.
  • A minimum of two or more years of progressive RN clinical experience in acute care or extended care nursing practice.
  • BLS, ACLS within 3 months of hire, PALS, TNCC, NRP completion within 1 year of hire.
  • Completion of a Gerontology Certificate Program or Resident Assessment Coordinator certification desired.

 

OTHER SKILLS & ABILITIES

 

Language Skills

 

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, patients, customers, and the general public.

 

Mathematical Skills


Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical solutions.

 

Reasoning Ability


Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
