JOB SUMMARY
In collaboration with the Chief Nursing Officer and the Nurse Manager, the Residential Care Coordinator is responsible for planning, implementing and over-seeing the care provided to Non-Skilled Swing Bed (NSS) residents. The Resident Care Coordinator (RCC) will be support the ongoing evaluation of the residents comprehensive plan of care. Uses established work systems and processes to facilitate quality of care and living environment, while ensuring ongoing regulatory compliance with all applicable laws and professional standards of practice. The RCC assumes nursing leadership functions by establishing workflow changes and performance improvement initiatives in the NSS unit. Facilitates effective communication of patient needs and issues to the interdisciplinary health team and fosters positive change to maintain a collaborative team environment.
ESSENTIAL DUTIES AND RESPONSIBILITES
Care Management :
- Serves as the primary communicator and support contact for the NSS Provider and family members.
- Completes the Resident assessment, intake documentation, care plans and other assignments to meet Federal and State regulations and quality expectation.
- Collaborates with CNO RN to conduct regular care plan review meetings.
- Works on the unit as an NSS nurse.
- Ensures appropriate staff are informed of all changes in condition and updated care planning needs.
- Manages scheduling, completion, and documentation of monthly Provider rounds.
- Manages scheduling, completion, and documentation of patient and family care plan meetings as indicated.
- Collaboratively works with residents, families, nursing staff, and Attending Provider in managing the care processes for each resident, ensuring that plan of care is assessed, reassessed and altered to meet evolving care needs of each resident, and each residents goals are clearly identified and that appropriate interventions and timelines are established and communicated to the team.

Language Skills
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, patients, customers, and the general public.
Mathematical Skills
Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical solutions.
Reasoning Ability
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
