Job Description:

Essential Duties and Responsibilities:
- Works as part of a multi-disciplinary team of the hospital staff and FGC staff responsible for assisting, monitoring and coordinating services for Medical Detox Patients.
- Assists patients with a variety of concerns, but the primary focus of the Medical Detox Unit is for patients to be bio-medically stabilized and to be referred to MAT and or a HLOC if requested.
- Assists in performing and completing a comprehensive bio-psychosocial assessments, the Lighthouse Locator and Admission and Discharge and The Government Performance and Results Act on each patient during shift.
- Assists with patient continued stay review process with the established procedure to monitor the medical necessity and appropriateness of continued hospitalization.
- Works collaboratively in formulating an appropriate and realistic discharge care plan, communicating with the patient. Completes appropriate forms, and contracts all appropriate individuals to facilitate the transfer/linkage of patients to other community based agencies and services. Ensures all referrals are linked through follow-up.

Schedule: Saturday - Sunday (6:00am - 2:30pm); hours may vary based on agency's needs
Start Date: ASAP
Pay Rate: Up to $18.00 per hour; based on experience
