About the Role :
The Case Manager - Behavioral Health-Substance Use Disorder plays a crucial role in supporting individuals struggling with detoxification and mental health issues. This position involves developing and implementing personalized care plans that address the unique needs of each patient, ensuring they receive the appropriate resources and support. The Case Manager will collaborate with a multidisciplinary team to monitor progress and adjust treatment plans as necessary, fostering a holistic approach to recovery. Additionally, this role requires building strong relationships with patients, empowering them to take an active role in their recovery journey. Ultimately, the Case Manager aims to enhance the quality of life for clients by facilitating access to essential services and promoting sustainable recovery outcomes.
We are currently seeking candidates with a Florida license in a related healthcare field such as a Certified Case Manager (CCM) or Nursing (RN) or (LPN)
Qualifications :
- Experience in case management or direct service provision in behavioral health or detox settings.
- Certified Case Manger (CCM), Registered Nurse (RN) or Licensed Practical Nurse (LPN) required
- Bachelor's degree in social work, psychology, or a related field preferred.
- Knowledge of substance use disorder treatment modalities and mental health resources.
- Experience working with diverse populations and understanding cultural competency in treatment.
Responsibilities :
This is a full-time position requiring availability Monday through Friday, with flexibility to occasionally work on a weekend day if needed.
