Job Overview
We are seeking a highly skilled Registered Nurse to lead our care management activities and ensure seamless access to quality healthcare services for our members with complex needs.
Key Responsibilities:
- Evaluate member needs, identify barriers to care, and assess social determinants of health to develop personalized care plans.
- Collaborate with healthcare providers, specialists, and community resources to address unmet needs and improve member outcomes.
- Coordinate with members, caregivers, and care provider teams to ensure timely access to necessary care or services.
- Monitor member status and progress towards care plan goals, making adjustments as needed to ensure optimal results.
- Facilitate care management and collaborate with providers to ensure seamless delivery of care services.
- Maintain accurate records of member information and care management activities to ensure compliance with regulatory requirements.
- Provide education to members and their families on disease processes, healthcare provider instructions, and care options.
Requirements:
- Bachelor's degree in Nursing from an accredited institution.
- 2-4 years of experience in care management or a related field.
- RN - Registered Nurse State Licensure and/or Compact State Licensure required.
