SUMMARY
The Assistant Case Manager will work closely with the Case Manager/Supervisor and provide support in an administrative capacity, working towards the goals and objectives of FIELD’s Work Experience (WEX) program. The Assistant Case Manager is responsible for the case management of all participants, keeping detailed records of various program components and assisting in preparing and maintaining participant case files to ensure participants receive the comprehensive services outlined by the Case Manager.
ESSENTIAL DUTIES and RESPONSIBILITIES include, but not limited to the following. Other duties and responsibilities may be assigned.
- Manage the placement, enrollment, and retention of disconnected youth aged 18-24 participating in FIELD’s work experience (WEX) program.
- Provide integrated case management and supportive services to participants of FIELD’s WEX program.
- Support in the daily operations and actively assist and provide direction to the FIELD WEX program team as needed.
- Conduct intake assessments with participants of FIELD’s WEX program to establish goals, interests, and services.
- Create individualized and comprehensive development plans for each participant.
- Support Case Manager in updating documentation with respect to participant case management, FIELD’s WEX program coordination, and collecting additional information, as needed.
- Contact FIELD’s Workforce Development (WFD) Managers for updated status reports or to answer questions directed from the Case Manager.
- Develop an effective structure for outreach and recruitment that is complementary to FIELD’s WEX program goals and objectives.
- Work with outreach coordinator(s) and case manager to identify and implement appropriate strategies aimed at improving work performance for participants.
- Track all FIELD WEX program participant referrals, assessments, demographics, community partnerships, retention, etc.
- Partner with other staff to update participant information in the project data collection system.
- Organize and schedule case management meetings with FIELD’s WFD Managers and staff to create or update participant plans, and to discuss performance evaluations.
- Ensure projects are completed on schedule following established procedures and schedules.
- Complete other projects and tasks as assigned.
- Prepare and submit daily, monthly, and quarterly reports.

- Proficient in Microsoft Suite (Word, Excel and PowerPoint).
- Must be able to learn about our organization, programs (EPIC, CTE, CCEC), Cesar Chavez history, and share this information with potential participants and others.

EDUCATION and/or EXPERIENCE
- Associate’s degree in a field related to counseling, psychology, social work or similar; 1-2 years of progressively responsible work experience preferred or any equivalent combination of education, training, and experience.
CERTIFICATES, LICENSES, REGISTRATIONS
Must have a valid driver’s license and personal automobile insurance.
TRAVEL REQUIREMENTS
Must be able to travel as needed/required.
LANGUAGE SKILLS
Bilingual in Spanish, required.
