We are seeking a Licensed Practical Nurse (LPN) – Case Manager for our Targeted Case Management (TCM) clients at our HOXIE office. This role is vital in assessing client needs, conducting in-home visits, ensuring compliance, and maintaining seamless communication between clients, caregivers, and our office team.

Key Responsibilities:
- Conduct in-home assessments and document client needs for personal care services.
- Schedule and complete regular home visits with clients.
- Input and manage client data using the company's Electronic Visit Verification (EVV) system.
- Maintain accurate records and ensure compliance with Medicaid, HIPAA, and company policies.
- Serve as a liaison between clients, caregivers, and the Office Supervisor to ensure quality care.
- Provide ongoing support and education to clients and caregivers.
- Maintain an organized and efficient office space.
- Adhere to all infection control policies, False Claims Act regulations, and privacy laws.
What We’re Looking For:
- Active LPN license in the state of Arkansas.
- Strong interpersonal skills with the ability to build meaningful relationships with clients and caregivers.
- Experience using Microsoft Office and Google Drive for documentation and reporting.
- Excellent written and verbal communication skills.
- Detail-oriented with the ability to manage multiple clients and compliance tracking efficiently.
- Ability to work independently, prioritize tasks, and meet deadlines in a fast-paced environment.
- Strong understanding of Medicaid and personal care program policies (preferred, but training will be provided).
- A commitment to maintaining confidentiality and handling sensitive information with discretion.
