What are the responsibilities and job description for the Social Services Coordinator - Intake and In-Home Visit Coordination position at First Choice In-Home Care?
First Choice In-Home Care is seeking a Social Service Coordinator to primarily be resposible for completing new Client Intake Appointments as well as current Client In-Home Visit Check-In Meetings. The position is based out of our corporate office location in Renton, WA.
First Choice is licensed by the Washington State DOH and contracted with DSHS to provide non-medical personal care to our population of developmentally disabled children/adults as well as aging individuals. We provide services to both State funded and Private clients living in King, Pierce and Snohomish counties. We are one of the largest In-Home Care Agencies in Washington State and manage the care of over 2,800 individuals throughout our service area. We are established, still growing and are now looking to hire a qualified, passionate and motivated staff member into the position ofSocial Services Coordinator.
Requirements and Qualifications:
1. 1 to 4 years of applicable experience; social services, healthcare or administrative settings
2. Excellent written and verbal communication skills
3. Proficient in the use of computers and Microsoft Office programs
4. eliable transportation and ability to utilize personal vehicle for travel to appointments (mileage reimbursement provided)
5. Ability to multi-task
6. Strong attention to detail
7. Excellent customer service skills
8. Hardworking and self motivated

Responsibilities:
1. Efficiently scheduling Client Intake and/or Client In-Home Visit Appointments
2. Conduct and complete Client Intake or Client In-Home Visit Appointments
3. Process associated paperwork, update Client records
4. Coordinate with Clients' assigned Social Services Case Manager, conveying pertinent updates from appointments
5. Develops and updates Client Plans of Care
6. Conduct thorough review of care services rendered
Hours: Full-Time hours from 8:30am to 5:00pm, 30min Lunch Break
