First Step Communities is a Sacramento based non-profit with the mission to create interim housing/emergency shelter and case management services for people experiencing homelessness.
There are currently 8 Case Management positions available. Additionally First Step Communities is looking for 2 additional Case Managers with SUD training and Community Health Worker experience.
Utilizing Trauma Informed Care, the Case Manager is responsible for providing client centered triage and system navigation services to clients enrolled in our shelters. The case manager will build rapport and trust with their clients while they assist clients in areas of increasing their income, employment skills, linkages to medical/mental health care, AOD services, crisis intervention, general case management including peer support, ADL’s, budgeting/finances and credit counseling.
- Ideally, the case manager should be comfortable approaching clients in a respectful and compassionate manner. The case manager should be confident in their ability to engage and motivate all of their clients. Including clients that might be experiencing a mental health crisis or struggling with active substance abuse.
- Professional boundaries are imperative in fostering an effective client-case manager relationship
- Must be able to motivate clients to achieve agreed upon milestones and be able to set clear expectations and boundaries.

Duties and Responsibilities: include the following; and other duties not specified.
- Effectively create a therapeutic relationship with clients utilizing Trauma Informed Care and Motivational Interviewing
- Assist individuals in identifying and removing barriers in order to end their current episode of homelessness.
- Assist clients with securing housing; completing applications, working with SHRA if a voucher holder, assisting client with housing waitlists, signing up for Ready to Rent classes, and any other pertinent skill/task in order to secure housing.
- Possess the ability to engage with challenging clients in a meaningful, respectful and client centered manner.
- Utilizing HMIS on a daily basis. Completing HMIS enrollments/exits, logging services provided, completing assessments and uploading documents within HMIS. Displaying integrity in data collection and reporting.
- Participation in the COC and interacting with CES in order to access available housing opportunities.
- The case manager will provide linkages to medical care, mental health services, employment or income services, AOD services, document acquisition, life skills or any other service identified by an individual.
- The Case Manager will assist their client in creating an agreed upon treatment plan with identified goals and timelines.
- Provide community referrals, information and support by linking clients to needed resources, advocate with and on behalf of clients when needed.
- Be able to provide basic educational skills regarding employment, resume writing, budgeting, credit repair, housekeeping and any other identified life skills.
- Maintain excellent documentation in client file
- Facilitate educational groups
- Observe and report any emergency situations to 911 or other appropriate emergency responder.
- Accurately complete all necessary forms, reports and paperwork required by the program on a timely basis.
- Support individuals with active listening, demonstrating empathy and by providing a message of hope and recovery.
- Escalate to Management’s attention and/or report any observed or suspected abuse and/or neglect of children, disabled individuals, dependent adults and elder/senior citizens.
- Regular, predictable attendance is required.
- Ability to get along and work effectively with others.
- Transport clients on an as needed basis in your personal vehicle (mileage reimbursed)
- Ability to work outside and/or in inclement weather as well as being able to walk on uneven surfaces/sidewalks.
Qualifications:
Education and Experience: Experience in social services: specifically with unsheltered individuals is highly desirable along with a college degree in a related field. The ideal candidate will be able to fulfill the above job duties and responsibilities.

- Ability to think quickly and respond calmly to stressful/ emergency situations.
- Able to work independently
- Ability to work in inclement weather, frequently walking, standing or stooping on uneven surfaces.
- Ability to use a laptop, computer or iPad and have a general knowledge of office equipment.
- Must have a valid drivers license, registration and insurance and be willing to transport clients as needed (reimbursement of mileage paid at $0.67/mile)

Pay range: $23-$27/hour
