JOB SUMMARY
The Client Care Coordinator will be responsible for designated areas of home care operations including, but not limited to new client intakes, service evaluations, writing care plans, and supervising care. The Client Care Coordinator will directly interface with clients and employees.
RESPONSIBILITIES
To perform this job successfully, the individual in this position must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the minimum knowledge, skills, and ability required. This position serves as a first touch point for new clients, and as an account manager for existing clients. In addition, the position plays a key role in identifying client needs and matching appropriate services.
- Conduct prospective new client intake calls, determine client needs, and schedule care assessments.
- Express proficiency in all FirstLight Home Care services, policies, benefits, and market differentiators.
- Responsible for prospect follow up process to include phone calls, emails, email campaigns
- Perform initial and ongoing in-home evaluation, caregiver introductions, coordination, and supervision of client services
- Create and maintain client care plans through the entry of accurate client data into FirstLight Home Care software systems
- Accurately identify and report changes in client condition to agency employees involved in the client’s care plan
- Drive company print and online review campaigns by successfully capturing client feedback into appropriate marketing channels.
- Participate in the quality assurance reviews and evaluations of the agency’s services
- Assist scheduler with scheduling of shifts by matching caregiver qualifications and availability to clients’ needs
- Supervise, coach and train caregivers to ensure all client care needs are being met.
- Work toward continuous quality improvement and provide unparalleled customer service for all FirstLight Home Care clients.
HARD SKILLS
- Experience working in home care, healthcare, or other senior-care-related industry required or an equivalent combination of education and work experience may be considered.
- Bachelor's Degree or higher in a related field is highly preferred
- CNA or Equivalent certification at minimum is required
- Proficient in assisting clients with personal care services (activities of daily living) and can demonstrate proper techniques and train others.
- Demonstrated experience in staff mentorship, training or supervision
- Working knowledge of the “senior-care-industry”, i.e. home care, home health and hospice, community care.
SOFT SKILLS
- Possess exceptional interpersonal skills - have a winning personality - managing positive outcomes for both clients and staff.
- Skilled at conflict resolutions - being able to quickly and effectively identify pain points, issues and implement solutions and problem solve.
- Self-motivated - must have the ability to work independently, maintain confidentiality of information and meet deadlines.
- Highly organized and able to prioritize responsibilities - managing necessary tasks and issues with a strong sense of urgency.
- Results driven - have the ability to independently take problems and create solutions - someone who takes personal ownership of their work.
