Reports To: Manager of Supportive Services
Position Purposes: The EFA/Intake Case Manager is responsible for managing Emergency Financial Assistance (EFA) applications and supporting clients at Five Horizons Health Services (FHHS) by addressing housing instability and financial barriers to care. This role involves conducting intake assessments, coordinating housing services, and ensuring clients have access to necessary social services, enabling them to maintain stable living conditions and remain engaged in HIV care. The EFA/Intake Case Manager works as part of an interdisciplinary team to identify and overcome barriers to care, promoting overall well-being and improving health outcomes.
EFA Responsibilities:
·Serve as the primary contact for clients requesting EFA assistance and / or receiving housing services.
·Maintain knowledge of non-clinical programs, resources, and services available to clients and facilitate linkage and / or referral as needed
· Collaborate with the Housing Opportunities for Persons with AIDS (HOPWA) Case Manager to coordinate services.
· Assess clients’ need for emergency financial assistance for housing and / or utilities and provide linkage to appropriate resources.
·Participate in additional training and educational opportunities to continually enhance knowledge and skills relevant to housing services, HIV Care, and client support
·Ability to engage effectively with diverse populations and cultures
·Maintain required client records, document services provided, scan client documents into Athena system, and complete other required paperwork in a timely manner
· Collaborate with medical providers, social workers, and other professionals to identify and address medical needs related to access / retention to care and treatment
· Ensure compliance with HIPAA, state regulations, and FHHS policies and procedures
·As with all other positions, during emergencies, the EFA/Intake Case Manager is expected to fill in at any position within the agency for which they can perform
Intake Responsibilities:
·Administer non-invasive HIV tests and provide pre- and post-test HIV education and counseling, ensuring clients are informed and supported throughout the process.
·Complete intake processes for new clients, including documentation of client needs and services requested. Engage in follow-up communication to ensure a smooth transition into care.
·Conduct walk-in intakes at FHHS and occasionally meet clients outside the office (e.g., home, hospital) to complete the intake process and gather necessary information.
·Communicate regularly with new clients to establish trust, ensure their needs are met, and encourage continued participation in care.
·Ensure compliance with HIPAA, state regulations, and FHHS policies and procedures.

Education/Professional
·Bachelor’s or Master’s Degree in Social Work, Human Services, Psychology, or a related field is preferred.
·Strong organizational, time management, verbal communication, and written communication skills.
Requirements: This position requires an individual who is self-motivated and able to work independently to complete assigned responsibilities. able to effectively communicate verbally and in writing. Individuals must be able to effectively utilize Microsoft Office programming and, after necessary training, an electronic health records system.
