Reports To: Manager of Supportive Services
Position Purposes: The HOPWA Case Manager is a compassionate, motivated, and committed professional that will help clients of Five Horizons Health Services (FHHS) achieve and maintain stable living environments to support engagement in HIV care. The HOPWA Case Manager is responsible for conducting housing assessments, providing supportive services, enhancing housing opportunities, and assisting clients with financial health and literacy. The HOPWA Case Manager will also develop relationships with community partners to support housing stability for clients of FHHS.
Essential Functions:
· Represent Five Horizons Health Services in the community and develop strong connections with local organizations and housing resources throughout the regional service area
· Serve as the primary contact for clients requesting HOPWA housing services.
· Perform home visits when required to ensure clients are meeting the requirements of their specified housing program
· Manage a caseload of clients receiving (HOPWA) Housing Opportunities for Persons with HIV/AIDS services, ensuring consistent follow-up.
· Participate in additional training and educational opportunities to continually enhance knowledge and skills relevant to housing services, HIV Care, and client support.
· Submit housing data reports in a timely manner, ensuring accuracy, and compliance with HOPWA guidelines
· Complete HOPWA client monthly and annual recertifications, ensuring all required documentation is submitted and up to date.
· Maintain knowledge of non-clinical programs, resources, and services available to clients and facilitate linkage and / or referral as needed
· Collaborate with medical providers, social workers, and other professionals to identify and address medical needs related to accessing HIV care and treatment
· Complete short-term and long-term housing applications to ensure clients remain stably housed
· Maintain required client records, document services provided, scan client documents into EMR system, and complete other required paperwork in a timely manner
· Maintain compliance with HIPAA and state guidelines as well as FHHS policy and procedures
· As with all other positions, during emergencies, the HOPWA Case Manager is expected to fill in any position within the agency for which they can perform
Education/Professional:
· Bachelor’s or Master’s Degree in Social Work, Human Services, Psychology, or a related field is preferred.
· Must possess a strong ability to interact with diverse groups and cultures
· Strong organizational, time management, verbal communication, and written communication skills
· Must possess a valid driver license and confirm effective automobile insurance coverage
Requirements: This position requires a self-motivated person who can independently complete assigned responsibilities. able to effectively communicate verbally and in writing. Individuals must be able to effectively utilize Microsoft Office programming and, after necessary training, an electronic health records system.

Pay: From $47,000.00 per year
