Position Overview: The PrEP Navigator at Five Horizons Health Services (FHHS) oversees PrEP (Pre-Exposure Prophylaxis), prevention, and medication adherence initiatives for the Southern Region. This role includes maintaining accurate patient records, monitoring appointment adherence, providing prevention education, and coordinating communication regarding medication deliveries from our contracted pharmacies. Additionally, the PrEP Navigator supports the clinic's by maintaining and organizing patient documents while ensuring compliance in accordance with the organization’s standards. This position encompasses clinical responsibilities related to patient education, adherence monitoring, and logistical coordination, enhancing the overall effectiveness and quality of healthcare delivery at FHHS.
Essential Functions:
- Provide PrEP adherence, education, and counseling patients.
- Maintain accurate records in the patient log.
- Monitor and reschedule missed PrEP lab and provider appointments.
- Coordinate with clinic staff and contracted pharmacies to ensure timely medication deliveries.
- Manage the patient mail-order medication list and follow up with patients for timely pickup.
- Assist in completing initial and annual renewal applications for Avita and Curant programs.
- Serve as backup staff support as needed, assisting with appointment scheduling, insurance verification, intake documentation, and clinical preparations.
- Collaborate effectively with clinic staff to ensure high-quality patient care.
- Support program goals and initiatives as an active member of the prevention team.
- Provide support to the Southern Region clinics for PrEP tracking.
- Work with Prevention and Outreach as needed to enhance linkage to care.
- Conduct sexual health risk assessments in order to determine level of risk and appropriate PrEP.
- Identify and mitigate patient barriers to PrEP access and retention to care.
- Provide stigma- and shame-free education and care at every interaction with patients.
- Maintain required records, files, and forms in an organized manner and in accordance with HIPAA and other state, local, and federal guidelines.
Additional Responsibilities:
- Report on PrEP and other applicable statistical information to the Director of Healthcare Operations monthly, and more frequently as applicable.
- Perform any other duties as assigned by the Director of Healthcare Services or Chief Healthcare Officer to support the clinic's mission and operations.
- Attend all required meetings and training (virtual or in-person) as designated by supervisors.

Qualifications:
- High school diploma or equivalent. Associate degree or higher preferred.
- Proficiency in Excel, Word, and Microsoft Windows.
- Knowledge of Electronic Health Records (EHR) preferred. Experience in clinical settings, especially in HIV prevention or related fields, is highly desirable.
- Strong organizational skills with meticulous attention to detail and the ability to prioritize tasks effectively.
- Excellent interpersonal and communication skills to interact effectively with diverse patients and third-party vendors, demonstrating a commitment to excellent customer service.
- Proficiency in using electronic medical record systems and basic computer applications.
- Ability to work independently as well as collaboratively within a team environment.

- This position operates primarily in a clinical office environment.
