SF Housing Services - Site S
The goal of Site S is to provide a warm, welcoming environment of support and services tailored to the unique needs of San Francisco residents currently experiencing homelessness. Site S is low-threshold with minimum rules and requirements in order to attract the hardest to serve individuals.
Up to 149 single adults without children who are experiencing homelessness will be accommodated at the SIte S shelter located at 1001 Polk Street in San Francisco. This shelter is based on harm reduction principles with the objective of providing intensive services leading to housing or treatment exits as quickly as possible. This environment will be unique in that it will be developing trusting relationships with people that have lived on the streets and dont usually enter the shelter. It will be a low threshold, minimal rule environment with a harm reduction focus.

POSITION SUMMARY
This Care Coordinator supports our guests with exit planning, obtaining the required documents for housing, and offers support while in the shelter. Their primary responsibility is to ensure coordination and placement to housing and follow-up post-housing placements. Care Coordinators will work with all partner agencies to exit participants into appropriate housing and/or treatment, providing smooth transitions and follow-up aftercare when necessary.

You will act as a listening ear, navigator, and at times advocate for guests who need extra support and maintain a safe, healthy environment for staff, guests, and their pets.

JOB DUTIES AND RESPONSIBILITIES
- Perform participant intake and assessments for 100% of Site S guests within 48 hours of entry into the hotel
- Work with guests to set goals and create Service Plans to get the treatment and/or housing ready
- Work with county and nonprofit partners to identify housing option for guests
- Provide referrals and linkages to community resources and benefits such as medical and mental health services on or off-site
- Provide follow-up support services for up to 3 months after guests are successfully housed to ensure a smooth transition into housing
- Perform wellness checks for guests who have serious physical and/or mental health conditions that require close attention
- Be able to quickly assess and respond to a variety of situations, including medical emergencies, conflict resolution and intervention, and other guest emergencies
- Regular communication with Director of Guest Services and Housing and Site Director on available housing, housing subsidies, and housing placement opportunities
- Collaborate and communicate with team members to identify guests that might be eligible for services and housing
- Perform goals tracking and guest charting
- To work with any and all interns or volunteers assigned to this site
- Share information and new resources with guests and staff
- Advocate on behalf of guests regarding property management issues, benefits issues, in connection with other outside agencies
- Maintain up-to-date documentation in guest files regarding services provided to guests, e.g. referrals, supportive services, in a complete, confidential, and professional manner
- Complete all necessary paperwork including monthly statistics, etc. in a timely manner
- In partnership with the Assistant Director of Navigation, follow and implement all policies and procedures of Five Keys Schools and Programs and the SIP hotel
- Establish and maintain a positive working relationship with employees, volunteers, and participants of the SIP hotels, other housing services providers, Five Keys, and with outside agencies
- Attend all meetings as assigned
- Other duties as assigned
KNOWLEDGE AND SKILLS
- Working knowledge and experience of housing services project planning and implementation
- Experience working in a SIP hotel or shelter having a full understanding of programmatic and administrative needs
- Critical thinking and problem solving

EDUCATION AND WORK EXPERIENCE
- Associates or Bachelors degree in Social Work, Psychology, Counseling or related field. or, extensive experience in this field
- At least two years paid work experience working with homeless adults with multiple disabilities or an Associates degree with five years paid related work experience
- Must possess a good working knowledge of and a sensitivity to individuals with substance abuse and/or mental health issues such as PTSD, psychosis, borderline behaviors, bi-polar, etc. and possess a working knowledge of crisis intervention techniques and be able to implement them
- Must possess a strong knowledge of social services; as well as mental health resources for low-income adults
- Must have excellent charting, record-keeping skills and assist in the development of a community resource list/file
- Must be computer literate
- Committed to the mission of The SIP hotels and Five Keys Schools and Programs and possess a good working knowledge of and a sensitivity to individuals with substance abuse and/or mental health issues such as: PTSD, psychosis, borderline behaviors, bi-polar, etc.
PHYSICAL REQUIREMENTS
