Branch Manager - Home Health - Chantilly

The Clinical Manager has accountability and responsibility for the appropriate and effective provision of Clinical Services to ensure appropriate outcomes and minimal re-hospitalizations in a cost-effective manner.

Summary of Duties

• Complete home visits as needed
• Complete HCHB Workflow
• Complete final QA review of all Oasis timepoints/485
• Review of discharge Oasis. Einstein to be sent for final approval by COS prior to locking
• Review/work Missed Visits and Outstanding Visits report
• Run/review LUPA report and make corrections as needed
• Review/approve or make adjustments to frequency requests in conjunction with Care Maps
• Monitor Forcura and Team chat
• Monitor clinical documentation submission
• Approve NVA for payroll

Qualifications and Skillsets:

• Requires graduation from an approved school of nursing, and an active unencumbered Virginia license as an Licensed Practical Nurse (LPN) or Registered Nurse (RN). BSN preferred but not required.
• 3-5 years’ transferrable healthcare experience; home health care management experience preferred.
• Good communication, negotiation and public relations skills.
• Autonomous, organized, assertive, flexible and cooperative.

Prefer: Technology expertise with field point-of-care solutions.

Prefer: Experience with Joint Commission home care standards.

Job Type: Full-time

Pay: $85,000.00 - $105,000.00 per year
