Job Title: Wayne County DSS Case Manager

Job Summary:

FLACRA is seeking a dedicated Case Manager to provide case management services for individuals seeking addiction treatment in Ontario and Wayne Counties. This role involves conducting assessments, offering therapeutic support, and connecting clients to essential resources, all in alignment with FLACRA’s mission, vision, and values.

Must-Have Qualifications & Skills:

• Associate’s Degree in Health or Human Services (or a related field).
• Valid NYS Driver’s License and access to reliable transportation for client visits.
• Excellent written and verbal communication skills.
• Ability to communicate effectively and work cooperatively with culturally diverse individuals, staff, and community service providers.
• Knowledge of local behavioral health systems.
• Strong multitasking, problem-solving, and time management skills, with the ability to remain calm in crisis situations.
• Experience with electronic health records (E-records).

Nice-to-Have Qualifications:

• Bachelor’s Degree in Health or Human Services (or a related field).
• Previous case management experience, particularly in addiction services.

Key Responsibilities:

• Provide direct diagnostic evaluation services to clients referred by various sources, assessing their needs for addiction treatment and additional counseling services.
• Deliver therapeutic services to clients through individual, family, conjoint, and group therapy sessions.
• Manage client intake processes and caseloads with independent judgment, seeking consultation as needed for diagnostic evaluations and treatment techniques.
• Participate in staff meetings, in-service trainings, and supervisory sessions to ensure alignment with agency goals.
• Build and maintain positive, effective relationships with community groups, referral sources, and service agencies.
• Engage in community presentations and training programs to raise awareness of addiction services.
• Assist clients in developing appropriate interpersonal, problem-solving, and decision-making skills through counseling and support.
• Collaborate effectively with coworkers and supervisors, demonstrating teamwork, flexibility, and cooperation.
• Respond appropriately to crisis and emergency situations to ensure client safety and well-being.
• Complete all documentation in a timely and accurate manner, in accordance with agency and OASAS regulations, policies, and procedures.
• Maintain a professional demeanor, providing high-quality service and care with respect and dignity for all clients.
• Perform other duties as assigned by the supervisor.

Pay: $17.00 - $18.50 per hour
