Job Summary
The Peer Provider is a vital part of the FLACRA team, delivering peer support services to individuals and families experiencing serious mental illness, substance use disorders, and homelessness . This role focuses on outreach and engagement for those who are homeless, ensuring continuous support until stable housing is secured .
Under the co-supervision of the Housing & Employment Manager and the Connections Regional Coordinator , the Peer Provider acts as a mentor, advocate, and guide , assisting individuals in navigating recovery, securing housing, and accessing community resources . This position emphasizes building relationships, removing barriers to recovery, and fostering independence within the community.

Required Qualifications
- Completion of NY state-certified peer support specialist training through OMH or OASAS.
- CRPA-P/CRPA and/or NYCPS Certification required.
- High School Diploma or GED required.
- Valid NYS Driver’s License required.
- Lived experience in mental health and/or substance use recovery is required.
- Must be 18 years of age or older .
- Strong interpersonal skills with the ability to build trusting relationships.
- Excellent verbal and written communication skills .
- Ability to multi-task, prioritize, and problem-solve in a fast-paced environment.
- Crisis intervention skills , with the ability to remain calm under pressure.
- Knowledge of peer support principles and the recovery-oriented system of care .
- Proficiency in documentation and data tracking .
- Comfort with community-based services, outreach work, and in-person engagement .
- Reliable transportation and a valid NYS Driver’s License required.
Key Responsibilities:
Outreach & Housing Support
- Engage individuals experiencing homelessness who are also struggling with mental health or substance use issues.
- Coordinate with CoC, DSS, and Coordinated Entry Systems to support clients in securing stable housing that promotes long-term recovery.
- Actively participate in treatment planning and client case discussions.
- Assist individuals and families in developing a recovery plan, including psychiatric advance directives and crisis planning.
- Provide psychoeducation, harm reduction strategies, and relapse prevention support.
- Serve as a role model, sharing lived experience to demonstrate resilience and coping strategies.
- Help initiate and sustain recovery by identifying support groups, treatment options, and self-care resources.
- Guide clients in overcoming obstacles to recovery, including transportation, employment, financial assistance, and medical care.
- Offer case management and resource coordination as an extension of support services.
- Maintain accurate documentation of client progress and engagement efforts.
- Advocate on behalf of individuals and families, ensuring they receive the necessary services and accommodations.
- Work closely with supervisors and treatment teams to discuss concerns and ensure coordinated care.
- Participate in required training sessions to maintain certification.
- Stay informed on crisis intervention techniques and early engagement strategies.
- Provide feedback to improve program effectiveness and community partnerships.
- Attend team meetings, supervision sessions, and professional development opportunities.
