Summary

We are seeking a Comprehensive Community Support Services Care Coordinator (CCSS Care Coordinator) to join our team at FMRS Health Systems, Inc. In this role, you will play a vital part in assisting our clients with navigating their mental health journeys. Your contributions will be essential in providing administrative support and ensuring that our clients receive the care they need.

This position is a member of a patient’s care team who works cooperatively with other care providers to ensure that the behavioral health services provided meets the patient’s needs. The Care Coordinator is a liaison between the consumer and other health care team members to make sure that the treatment plan put in place by the care team is working effectively for all involved. The Comprehensive Community Support Services Care Coordinator works exclusively with CCSS consumers and participants in Supported Employment services. The CCSS Care Coordinator develops plans to specifically address client training deficits.

Responsibilities

  • Receives and provides “warm hand-offs” for consumers needing to be linked with CCSS or Supported Employment services.
  • Reviews consumer functional assessments and develops skill building plans as appropriate.
  • Reviews skills, connects to resources, educates patients on mental health and substance use disorders and treatment, and facilitates treatment plans.
  • Trains Community Instructors on skill building plans for consumers and monitors implementation in the classroom and community providing feedback to the Community Instructors.
  • Shares communication between FMRS providers (i.e. therapists and medical staff) and outside providers to facilitate and improve continuity of care. Makes referrals to outside providers as appropriate.
  • Calls consumers who have been referred to resources to schedule appointments and to improve follow-up.
  • Conducts initial clinical evaluations and reassessments to determine diagnostic impressions and make appropriate treatment recommendations as needed to support the assessment team.
  • Provides Care Coordination and Targeted Case Management as appropriate for CCSS participants.
  • Conducts outreach within the community to create a list of business partners for Supported Employment services.
  • Completes all documentation services in a timely manner in accordance with current procedures and processes.
  • Monitors alerts in EMR (failed activities, missing treatment plans, activities not statused, etc.) and resolves issues daily.

Requirements

  • Bachelor’s Degree in a human service related field (psychology, social work, sociology).
  • Experience with behavioral health or substance use treatment preferred.
  • Valid WV driver’s license in good standing.
  • Must be able to pass all required background checks.

Pay: $19.23 per hour

Expected hours: 40 per week
