FOCUS JOB DESCRIPTION
Position: Community Support/Direct Support Staff
Hours: Varies – Day, Early Afternoon
Supervisor: Regional Manager and Program Manager's
Job Purpose: The Direct Support Staff position is responsible for performing essential tasks assisting individuals with disabilities and/or behavioral challenges with everyday lives and independence.
Job Duties: Primary Duties include but not limited to
Review and complete sign off sheet for individual ISP, BSP, and supervision levels before working with individual
- Responsible for meeting utilization hours and making up any hours missed
- Assist in getting shift coverage for any time off
- Assist covering shift for other staff
- Responsible for addressing goals/objectives daily
- Attend ISP meeting upon request
- Responsible for the data collection related to objectives, behavioral support plans, and skill building plans
- Responsible for the daily health and safety of the individual
- Responsible for all duties related to direct-care and Activities of Daily Life to include lifting items like groceries, walking stairs, getting in and out of vehicles
- Communicate incidents to the proper parties
- Transportation of individuals to and from activities as needed or help individuals achieve independence through public transportation
- Documenting and reporting progress to support team and Employment Specialist
- Help individuals achieve independence in their workplace by managing and achieve behavioral objectives
- Assist individuals in achieving independence by creating checklists and other reasonable accommodations by working with the individual's supports.
- Maintain communication with Program Manager and treatment team
- Responsible for adhering to the Policies and Procedures of the Company
- Utilizing EVV/Setworks for clocking in and out of sessions
- Completing notes and documentation within same day of shift
- Utilizing company provided email and checking on a daily basis
- Attending a monthly staff meeting
- Providing HR with appropriate documentation in a timely manner
- Completing all assigned monthly trainings before the 30th of each month

- May also need to assist individuals with getting up, walking, and moving, by lifting or guiding when necessary.
Skills/Qualification:
Have a valid driver’s license, registration, insurance, and inspection
- Professional technology skills
- Good written and oral communication skills
- Knowledge of computers
- Strong organizational skills
- Ability to multitask and prioritize
- Ability to establish and maintain effective working relationships with groups and individuals
- Ability to maintain current needs assessment information
Compensation: Hourly rate (based on experience) plus mileage reimbursement
Bonus payments are contingent upon meeting new hire expectations for submission timelines and performance.
