Responsibilities
- Collaborate with multidisciplinary teams to develop comprehensive treatment plans tailored to each client's needs.
- Links clients with external programs or services, such as: health services, recreational activities, child care services, financial resources, housing placement, employment resources, child care, AA/NA groups, etc.
- Prepares written service plans and treatment update summaries, incorporating input from clients, other service providers, wraparound team members to identify problem areas and needs, service objectives, and intervention strategies
- Maintain accurate documentation of client interactions, treatment plans, and progress notes.
- Participate in ongoing training and professional development opportunities to enhance therapeutic skills.
- Attends bi-weekly staff meetings, group supervisions, and/or trainings
- Development of an Individualized Service Plan based on the problems and needs identified during the assessment and agreed upon by the provider and individual.
- Review of the individualized service plan with the individual at least quarterly.
- Ongoing revisions of the ISP as treatment needs change will serve to keep the ISPs current and a complete review and revision will be completed at least annually.
- Provision of training and reinforcement of functional skills and appropriate behavior related to the individual’s health and safety, activities of daily living, and use of community resources; assistance with medication management, and monitoring health, nutrition, and physical condition.
- Maintain timely and accurate documentation of service provision.
Experience, Skills and Abilities
- At least two years of clinical experience providing direct mental health services to persons who have a psychiatric diagnosis
- The employee must have criminal background check, maintain a valid driver’s license and have a satisfactory driving record from DMV. Knowledge of the principles, techniques and practices related to the provision of community based mental health community support services
- Knowledge of casework documentation and requirements to assure compliance with regulatory requirements
- Knowledge of mental illness and therapeutic interventions
- Knowledge of community resources and entitlement benefits
- Ability to develop and implement appropriate individual service plans
- Ability to interact with referral sources and other community service providers
- Ability to utilize clinical supervision to improve service delivery and documentation
Education:
- The MHSB QMHP-A will have at minimum an Associate’s Degree in a human services or related field (social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation, human services counseling or other degree deemed equivalent to those described) and meet the qualifications for QMHP
- And/or at least three years of clinical experience providing direct mental health services to persons who have a psychiatric diagnosis.
- Has a minimum of 90 hours classroom training, 12 weeks of experience of the direct personal supervision of a QMHP-A providing services with mental illness and at least one year of experience (including the 12 weeks of supervised experience)
