Qualified Intellectual Disabilities Professional (QIDP) I/DD Programming and Activities

About the Opportunity: The Qualified Intellectual Disabilities Professional (QIDP)
disabilities ensures the safety, health, and well-being of our residents. The QIDP is responsible for the collaboration between all individuals pertinent to the life of the individuals served. The QIDP develops and implements annual comprehensive program plans; reviewing all data and objectives monthly and routinely develops quarterly reviews. The QIDP is responsible for the overall monitoring of active treatment within the facility including ADL’s, social skills, mealtime skills, money management, medication administration and more.

Principle Roles & Responsibilities:
The Qualified Intellectual Disabilities Professional (QIDP) will be responsible for coordinating and/or chairing the following meetings:
1. Interdisciplinary Team (IDT) plan meetings regarding individuals on their assigned caseload. 2. Scheduling and chairing all intake and discharge team meetings, special team meetings, and periodic team reviews. 3. Scheduling and chairing meetings after individual discharge from hospitalizations when changes in programming are necessary. 4. Develop, plan, coordinate and deliver activities/outings to individuals with intellectual/developmental disabilities.

The Qualified Intellectual Disabilities Professional (QIDP’s) responsibilities with regard to consultant activities includes:
1.Ensure individuals are available and staff are scheduled at the assigned times for scheduled assessments.
2. Determine all needs for assessments in order to complete a comprehensive functional assessment tool (CFAT).
3. Report to the Sr. QIDP one week prior to the plan meeting any consultant reports that have not been received, including any in-house assessments.
4. Responsible for notifying the appropriate consultant when current programming is failing to meet the individual’s needs, or when there is a substantial change in the individual’s status which could require additional evaluations.
5. Assist consultants to ensure they have the tools needed to complete all job duties.

The Qualified Intellectual Disabilities Professional (QIDP) is responsible for the following monitoring and supervisory roles:
1. Complete monthly progress notes for each individual in their caseload by the seventh day of each month.
2. Review documentation books on a weekly basis to ensure that documentation is complete. Review missing documentation with Team Leader.
3. Fill in for absent Facility Supervisors; providing all aspects of client care.

The Qualified Intellectual Disabilities Professional (QIDP) will monitor and assure the following:
1. Follow a personal schedule which conforms to the expectations of the facility.
2. Monitor at least two meals per week by silently observing in and out of the way location; ensuring compliance with meal card directions, mealtime objectives, and proper use of mealtime adaptive equipment.
3. Monitor all timelines including mealtime schedules, active treatment schedules, objective completion dates, etc.
4. Monitor the cleanliness, sanitation, and organization of all workspaces.

The Qualified Intellectual Disabilities Professional (QIDP) will ensure the following standards are met:
1. Person Centered Plan development which includes the individual, legal guardian, service coordinator, direct care staff, and Interdisciplinary Team.
2. Documentation, disbursement, and implementation of all Person-Centered Plans on the same date of the plan meeting.
3. Ensure all formal objectives are written in a manner that complies with state regulations (W229 through W237).
4. Ensure documentation is implemented and completed in a manner that complies with state regulations (W252 through W254).
5. Review data, objectives, and plans monthly, revising, when necessary, based on state regulations (W255 through W258).
6. Develop and maintain excellent working relationships with guardians, family members, community providers, co-workers, and administration.
7. Maintain excellent attendance with no tardiness.
8. Perform other reasonably related duties as assigned.
9. Valid driver’s license and a good driving record required.

Educational Requirements: The QIDP must hold at least a Bachelor of Science or Bachelor of Arts degree from an accredited college in a Human Services related field.

Experience Required: The Qualified Intellectual Disabilities Professional (QIDP) must have at least one year of experience providing habilitation to individuals with intellectual/developmental disabilities. Experience writing individual program plans/future planning preferred. Supervisory experience is required. QIDP experience in an Intermediate Care Facility setting preferred.

Skills and Knowledge /Professional Attributes: The Qualified Intellectual Disabilities Professional (QIDP) must possess excellent communication and assessment skills. Working knowledge of behavioral sciences and affiliated disciplines related to evaluation and habilitation of individuals with developmental disabilities. The Qualified Intellectual Disabilities Professional (QIDP) must be a team player and proven leader. Must be able to handle stressful situations physically and mentally and react quickly and effectively. Must be able to work with a diverse group of people, make decisions, be well organized, flexible, self-motivated, reliable, enthusiastic, and creative. Must be able to maintain a pro-active stance in all areas of employment. The Qualified Intellectual Disabilities Professional (QIDP) maintains a positive and professional image and attitude; providing individuals served with a positive role model regarding appearance, personal conduct, and the use of manners both in the facility and in the community.
