What are the responsibilities and job description for the Baylor Plan/Weekend Package - Certified Nurse Assistant (CNA) position at Folkston Park Care & Rehab?
Folkston Park Care and Rehab is hiring a CERTIFIED NURSE ASSISTANT (CNA) to work providing both short term rehabilitation and long-term care on the BAYLOR PLAN/WEEKEND PACKAGE.
The Every Weekend Package/Baylor Plan requires a CNA/LPN/RN to work every weekend each month. This is a full-time or part-time position depending on the hours worked.
The parameters for the program are as follows:
•Hours worked:
o CNA/CMA.LPN/RN works 32 hours (2x16), will be paid an 8-hour bonus = 40 hours
o CNA/CMA/LPN/RN works 24 hours (2x12), will be paid a 6-hour bonus = 30 hours
• Works every weekend
• CNA/LPN/RN may request off 1 weekend a quarter (4 weekends per year total).
• CNA/LPN/RN may request off 1 holiday per year.
• CNA/LPN/RN is not eligible for any shift differential.
• If a CNA/LPN/RN calls in absent or are tardy for a shift during the pay period, they will forfeit the Weekend Package/Baylor Plan payments for that entire pay period.
• If a CNA/LPN/RN fails to punch in or out for a shift or needs to complete a missed punch form, they may forfeit the Weekend Package/Baylor Plan payments for that entire pay period. If they are paid for the Weekend Package/Baylor Plan, that payment will be delayed.
• If a CNA/LPN/RN calls in absent for more than 3 separate shifts, they will no longer be eligible to work the Weekend Package/Baylor Plan.
The rate for the CNA/LPN/RN will be determined by your hire in rates based on position and years of experience.
Applies only to Saturday and Sunday shifts:
•2 x 12-hour shift = 24 hours, will be paid for 30 hours (Part Time)
•2 x 16-hour shift = 32 hours, will be paid for 40 hours (Full Time)
