What are the responsibilities and job description for the Admissions Navigator position at Fora Health?
ESSENTIAL JOB FUNCTIONS
- Gather pertinent information from prospective patients, referents and providers to ensure that all required information is obtained.
- Address patient questions regarding Fora services, enrollment processes and insurance coverage.
- Responsible for gathering and maintaining organization of incoming referrals and requested documentation.
- Provides an overview of expectations for admissions process to patients and support persons.
- Communicates referral requirements and admission criteria to community partners and individuals seeking treatment.
- Enter patient information into customer relation software.
- Outreach to patients without outstanding items needed to complete referrals.
- Submit records requests for patients attempting to access treatment services.
- Assist patients through the enrollment process and informing patients of alternative resources.
- Conduct screenings for potential patients.
- Coordinates outreach attempts through calls, texts and emails to prospective patients.
- Answer calls and field questions regarding all Fora services and assist patients accessing services at all levels of care within Fora.
- Assist patients with accessing resources such as bus tickets, honored citizen applications and outside community resources.
- Provide de-escalation and crisis mitigation tactics in person and over the phone.
- Communicate directly with potential patients and referents via phone and email.
- Communicate and coordinate with staff from other departments effectively.
- Provides back up coverage for daily admissions tasks such as collecting urine drug screens, signing paperwork, reviewing patient belongings and scheduling intakes.
- Communicates with residential staff regarding patient arrival onto the residential floor and provides support with preparing patient rooms when needed.
- Track data related to priority/high risk groups.
QUALIFICATIONS
- Excellent organizational skills and attention to detail
- Work knowledge or experience working in behavioral health field
- High school diploma or equivalent
- Basic knowledge of substance use disorders strongly preferred
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
- Strong organizational skills and attention to detail
- Maintain confidentiality of sensitive information and documents
- Proficient in Microsoft Office applications
- Communicate information effectively in writing and verbally
ADDITIONAL JOB RESPONSIBILITIES
- Attend meetings as assigned
- Special projects and other duties as assigned.
- Maintain and organize file structure.
SUPERVISORY RELATIONSHIPS
- This position has no supervisory duties.
OTHER REQUIREMENTS
- Verification of Sobriety, per Oregon Administrative Rules (OAR) standards
- DHS Background Check Approval
- Successful completion of Drug Test upon hire
- Documentation of Tuberculin test and/or evaluation with negative results or evidence of non-communicability
WORKING CONDITIONS
- This position generally works in an indoor office environment
- Possible exposure to communicable diseases which can encompass a variety of illnesses and infections, including tuberculosis, Hepatitis A, B, and C, and HIV.
- Possible interactions or contact with people experiencing a wide range of emotions, presentations, abilities, social identities, mental states including adolescents, family members and support people.
