POSITION SUMMARY
The MAT Medical Case Coordinator will provide support for the medical team, with core responsibilities  within our Medication As Treatment (MAT), Ambulatory Withdrawal Management, and Psychiatric programs. They will be part of a multidisciplinary team that includes medical providers, nurses, residential assistants, addiction counselors, peer mentors, and administrative staff.
The Coordinator will coordinate evidence-based interventions to Fora patients seeking medication to treat their use disorders and provide general administrative support to the medical and psychiatric department. The Coordinator should have a kind, non-judgemental and curious attitude with willingness to learn about harm reduction, trauma-informed care and motivational interviewing. They must be comfortable coordinating and providing outreach and care transition services to people struggling with substance use and mental health conditions. Essentially, the coordinator is the “glue” for the program. 

ESSENTIAL JOB FUNCTIONS
  • Conduct screenings, obtain releases of information (ROIs), communicate with patients and other healthcare providers, communicate with community partners, schedule patients within the interdisciplinary teams and provide follow-up with patients, sometimes as a primary contact.
  • Make referrals all levels of care by providing clinical information and documentation to support patient transitions of care to appropriate services.
  • May include scheduling intake appointments, arranging follow up appointments, assisting patient with obtaining PCP.
  • Provide access to harm-reduction/recovery/treatment services for people seeking out assistance, regardless of pathway to recovery. 
  • Attend community meetings and promote program awareness and outreach.
  • Facilitate a smooth and pleasant transition between all level of care at Fora Health or external levels of care.
  • Work in conjunction with and under the supervision of the Medical Operations Supervisor and work collaboratively with Fora Health’s systems of care at other locations.
  • Support transitions between levels of care and places of service within Fora Health and continuing the relationship throughout the outpatient episode transitions.  This will also be achieved by developing strong working relationships with staff of other departments.
  • Facilitate all steps in patient care for MAT and Psychiatric services including managing appointments, troubleshooting with pharmacies, and supporting referrals to community-based services, in collaboration with MAT Hillsboro Coordinator
  • Coordinate smooth transitions for patients through Fora services, such as adding MAT to residential or outpatient (OP) services.
  • Provide outreach for entry into episodes of treatment within the community.
  • Have a strong understanding of intercultural values, strengths and needs and able to use this understanding of various cultures to increase access to services of treatment and increase potential of success. 
  • Assist patients in accessing resources in the community.
  • Assist patients with obtaining indigent funding if needed.
  • Follow agency policies and procedures provided by Fora Health, including but not limited to: Fraud, Waste and Abuse, Conflict of Interest, Boundaries, Ethics, Confidentiality, Drug-Free Workplace and Harassment Policies.
  • Collaborate with milieu counselors to provide and maintain a therapeutic environment with the patients at all sites.
  • Attend trainings and meetings as scheduled and required by supervisors.


SUPERVISORY FUNCTIONS
  • This position has no supervisory duties.


ADDITIONAL JOB RESPONSIBILITIES
  • Special projects and other duties as assigned
  • If deemed eligible to drive a company car, do so responsibly, with safety being the first priority and maintain your clean driving record to continue in this capacity.
  • Must be comfortable working as part of an interprofessional team.


QUALIFICATIONS

EDUCATION AND EXPERIENCE REQUIRED
  • Bachelor’s Degree preferred
  • 3 years relevant experience in healthcare or behavioral health discipline areas, preferentially with substance use disorder and underserved population experience. Relevant experiences include but are not limited to  care coordination, case management, treatment of substance use disorders, community mental health, chronic illness management, patient education, and complex social issues.

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED
  • Demonstrate genuine concern for people struggling with substance use disorders and the ability to treat all people and agencies with dignity and respect.
  • Work effectively within a team environment and be willing to receive and act upon feedback.
  • Relate to patients, families and staff  in a manner which is open-minded and non-judgmental in relation to individual differences and diverse backgrounds integrating biculturalism.
  • Stay calm, alert, and emotionally available in a high-risk, general public environment, effectively de-escalating as needed.
  • Maintain appropriate professional boundaries in working with all others (patients, families, staff, referents, etc.).
  • Demonstrate strong verbal communication and active listening skills.
  • Creatively problem solve and serve as a resource (or know where to find resources) to patients and families.
  • Demonstrate initative, self confidence, and serve as a role model to patients encouraging positive and pro-social communication and advocacy for individual needs.
  • Maintain self-care practices and develop new practices.     

OTHER REQUIREMENTS
  • Verification of Sobriety (if in recovery), per Oregon Administrative Rules (OAR) standards
  • DHS Background Check Approval 
  • Successful completion of Drug Test upon hire
  • Documentation of Tuberculosis test and/or evaluation with negative results or evidence of non-communicability


WORKING CONDITIONS
  • This position works both in an indoor and outdoor environment with frequent travel between sites or to special events and engaging with community partners at their facilities. 
  • Position requires some overtime, including evening and weekend work.  Adjustment of work schedule may be required.
  • Possible exposure to communicable diseases which can encompass a variety of illnesses, viruses and infections, including tuberculosis, Hepatitis A, B, and C, and HIV.
  • Possible exposure to upset, angry, severely traumatized, disabled or emotionally disturbed adults, adolescents, and families.
  • Exposure to potentially hazardous cleaning chemicals, personal protective equipment provided. The MAT Medical Case Coordinator assures that this mission is the foundation for the delivery of services and carrying out job duties.
