Community Health Representative
This position is partially or fully grant funded. Employment beyond the term of the grant award may be subject to approval by Executive Council.
About the job:
The Community Health Representative program is designed to provide support, advocacy and personal care to families with children with autism or significant special needs in the community. The goal is to maximally support the development of each child and the functioning of each family, and connecting families to financial, emotional, physical and spiritual resources within a culturally safe framework.
Here’s what you’ll be doing:
- Provides home and office visits to community members with an emphasis on maximizing each child’s potential and the health of the child and family (short term respite of a few hours a month with adult family member in household, helping to implement home treatment recommendations, helping to secure resources to maintain a safe physical environment for the child and family, building relationships with families).
- Supports the implementation of care plans, as developed by the child/family and care team.
- Serves as a liaison between the medical/community health/behavioral health staff, family services and other tribal programs on behalf of clients needing assistance.
- Identifies the appropriate resources necessary to provide solutions to problems encountered by clients in obtaining recommended services as indicated by medical diagnosis, physical condition, or social or spiritual need.
- Coordinates transportation to health care facilities or tribally sponsored events with adult family member of child.
- Documents all client visits into appropriate documentation systems following the CHD documentation policies. Ensures confidentiality of all records and information.
- Works closely with other team members such as Case Managers, Medical Providers, Behavioral Health, Birth to 3, School Staff and Community Health Nurses to maximize child and family health.
- Participates in the Quality Improvement and Public Health Accreditation activities as requested.
- Attends meetings, trainings, events and in-services that may require occasional evening, overnight or weekend hours as assigned by supervisor.
What you’ll need to be successful:
- High School Diploma or GED
- At least 2 years of experience doing direct work with young children in a formal teaching, childcare or clinical capacity
- Familiarity with social service, school and health care systems
- Must have and maintain a valid driver’s license and be at least 18 years of age
- Must successfully pass all applicable background checks and drug screens
- FCP complies with all CDC vaccine and testing recommendations for healthcare professionals.
Preferred qualifications:
- Early Childhood Educational Certification or CMA, EMT or LPN certification preferred.
