Job Overview
This position provides Social Work functions to the Maternal Infant Health program (MIHP), Healthy Start, In-Home Services program and Hannahville Health clinic patients. Maternal Child Health programs: Conduct home/office/community visits, provide education/support/resources, attend multidisciplinary team and community meetings, compete scheduled chart/billing audits, create/update policies and procedures, attend mandatory grant and Medicaid program meetings, complete quality assessments, maintain updated documentation and reporting requirements. Adult and Elder Programs: Conduct in-home assessments, create care plans, provide education/support/resources, maintain loan closet, attend multidisciplinary team and community meetings, attend mandatory grnat meeting, create/update policies and procedures, review daily service logs, create monthly billing spreadsheets, maintain updated documentation and reporting requirements. communicate with Life Alert company(s), provide cleint referrals for units, and maintain updated client information. May transport clients as needed. Upon request, provide coverage in the Community Health programs. provide basic CPR and/or first aid in cases of emergencies and assistance during emergency situations. All other duties as assigned or required to complete job duties, including varying grants. Must possess current MI. licensure (LLBSW or LBSW) and have a Bachelors of Social Work (BSW) degree. Possess and maintain valid MI. driver's license (in compliance with HIC insurance carrier) and as well as a licensed, insured vehicle for use on the job. CPR certified or ability to become CPR certified within 6 months of hire. One year post-licensure experience ro three years post-graduate experience prior to licensure in working with families preferred. Home visiting preferred. Knowledge of tribal culture, maternal and child health (pregnancy and infants); aging processl local resources and programs.
