SUMMARY
Assesses and evaluates the health status of resident / patient and provides care and treatment in accordance with physician orders, standards of practice, and federal guidelines.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Upon admission, assesses patient / resident by physical examination including wound measurements, head to toe assessment, pertinent diagnostic testing, etc. to determine health status
- Reviews patient / resident records for general health status to include risk factors
- Completes required forms and documentation to include admission assessment, physician orders, and all admission documentation in accordance with company policy and state / federal regulations
- Communicates with physicians and responsible parties / family regarding admission status and physician orders
- Educates patient / resident and / or responsible party / family regarding pertinent health status upon admission and plan of treatment
- Provides educational opportunities to staff regarding resident / patient care needs based upon general status and diagnosis
- Coordinates and assists in the patient / resident admission process
- Performs admission criteria per facility policy
- Provides and documents patient / resident care based on nursing process in compliance with facility standards and state and federal guidelines.
- Utilizes basic physical and psychological assessment skills to identify clinical concerns for each resident / patient. Implements preventive measures for each concern.
- Adheres to scope of practice limitations based upon qualifications
- Will adhere to infection control standards based upon company policies and CDC guidelines
- Will perform general nursing duties in cases of emergency or staffing shortages as assigned by DON
ENTRY QUALIFICATIONS
SUPERVISORY RESPONSIBILITIES
May supervises nursing staff and / or others for whom they are administratively or professionally responsible. May complete annual evaluations for potential merit increases for direct reports.
PHYSICAL DEMANDS AND ENVIRONMENT
Shift work may be required. Employee is frequently required to stand, walk, use hands and fingers, reach with hands and arms, feel, talk, and hear. Employee will occasionally assist with lifting and transferring of residents / patients weighing between 100-250 lbs. Employee is occasionally exposed to blood and / or other body fluids, fumes or airborne particles, and toxic or caustic chemicals. May be required to respond to critical facility issues on a 24 / 7 basis. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
