JOB DESCRIPTION
This is a Case Management position working with individuals diagnosed with an intellectual and/or developmental disability, children and adults. This is a full-time position with a regular but flexible schedule. The Case Manager will be expected to carry a minimum caseload of 25 individuals. The position entails assisting these individuals in gaining access to and receiving, needed social, medical, education, vocational, residential and other services as needed per the individuals plan of care. The Case Manager will be expected to complete monthly monitoring of the services that each individual on their caseload receives as well as the individual’s health, safety and welfare. A monthly face to face visit with each assigned individual is required. We serve all nine counties of region one. This includes: Ballard, Calloway, Carlisle, Fulton, Hickman, Livingston, Marshall, Mayfield and McCracken. Travel is required and is reimbursed.
Major Tasks/Responsibilities
· Case Managers are responsible for completing comprehensive assessments annually as required by funding sources.
· Facilitation of team meetings annually, or as needed, to develop person centered service plans.
· Arranges for and assists assigned individuals in accessing needed services and resources.
· Develops and maintains professional and supportive relationships with consumers, guardians, affiliate agencies and, as appropriate, other relevant parties.
· Advocates on behalf of assigned individuals and enhances natural support system.
· Follow-up on all voicemails and emails within 24 business hours.
· Be able to complete thorough and timely documentation of all services provided. Be comfortable documenting in and utilizing multiple electronic medical records.
· Assists in crises and crisis planning and serve on the D/ID crisis on-call rotation.
Qualifications/Requirements
· Bachelors’ degree in a human service field.
· Valid driver’s license and car insurance.
· Must pass all initial and annual background checks.
· Must pass drug screening upon hire and annually as required by funding source.
