SUPPORT BROKER/CASE MANAGER, IDD
REPORTING/COORDINATING RELATIONSHIP: IDD Coordinator of PDS Services
SUPERVISORY RESPONSIBILITIES: None
PRIMARY RESPONSIBILITIES:
The Support Broker will ensure that participants are referred to the necessary combination of services to maintain their placement in the community whenever possible. When not possible, the Support Broker will assist in applying for other services.
Specific tasks include but are not limited to the following:
(1) Schedule and complete evaluations/assessments yearly to assess participant’s needs.
(2) At one year intervals review POC’s, meet with participant, families, service providers, and guardian(s). The Support Broker should be able to guide the writing of outcomes and objectives to ensure clarity, measurability, and applicability to the participant.
(3) Fill out or assist in filling out any applications for needed services and equipment.
(4) Be able to write clear case notes in the format approved by funding sources and submit them electronically.
(5) Maintain a target number of direct client services and/or clients served as designated annually by the IDD Director.
(6) Develop Person-Centered Plans for participant and submit paperwork electronically.
(7) Be available to participant 24/7 if a need or question arises.
(8) Conduct home visits monthly monitoring participant’s budget, health, safety, welfare, and ensure required regulations are being meet.
(9) Monitor KY Health Net to ensure participants maintain Medicaid eligibility.
(10) Monitor and approve participant’s employees timesheets submitted via EVV (Electronic Visit Verification).
(11) Assist PDS Employees with any issues or errors that may occur in the EVV Mains’l System.
(12) Other duties as required/assigned.
There may be periodic travel in other counties in the eight-county region. The primary client group will be Children and/or adults with intellectual disabilities or developmental disabilities.
Residence in one of the assigned counties in the region is preferred.
MINIMUM QUALIFICATIONS:
Minimum of a bachelor's degree from an accredited institution in a human services field is preferred. Other qualifying degrees include, but are not limited to the following: registered nurse, licensed practical nurse, licensed clinical social worker, licensed marriage and family therapist, licensed professional clinical counselor, licensed psychologist, or licensed psychological practitioner. Must pass required training within specified timeframe. Must have current driver's license, proof of automobile insurance, reliable transportation, and be able to travel throughout the region. Must present proof of a recent (less than 1-year-old) TB skin test, and must pass a legally required Criminal Record Check, Central Abuse Registry check, Kentucky Nurse Aid Registry, and Misconduct registry.

Pay: $15.00 - $16.00 per hour
