General Summary: The Pharmacy Community Health Worker (CHW) plays a key role in supporting patient access to medications and health resources within our health center. This position focuses on assisting patients with medication access, including Medicare navigation, mediation adherence support, and coordination of prescription deliveries between our various facilities. The CHW will work closely with the pharmacy team, clinical staff, and community resources to enhance patient care and improve health outcomes.
Responsibilities:
Pharmacy & Patient Support
o Crosstrain as a Pharmacy Technician
o Assist with medication access, adherence, and cost-saving resources
o Guide patients through Medicare Part D and prescription assistance programs
Medication Delivery & Coordination
o Deliver medications securely between clinic sites and patient locations
o Maintain delivery records and ensure compliance with pharmacy regulations
SDoH Screening & Referral
o Assess patient needs related to insurance, food, housing, employment, healthcare, and transportation
o Refer patients to social, medical, and pharmacy services
Community Outreach & Education
o Participate in community events to share pharmacy services
o Provide education on medication management, health conditions, healthy behaviors, and chronic disease prevention
