POSITION SUMMARY
The Primary Care Family Nurse Practitioner (APRN) or Family Practice Physician functions as a member of a medical team and applies to the treatment of patients the medical and related skills acquired through the training and / or experience required for this position. This position may provide services within one or more Wheeler programs, and the concentration between programs may change with the needs of the organization.
This position will serve the full life span.
LOCATION
Plainville, CT, as well as coverage other health center sites (Waterbury, Hartford, Bristol, New Britain).
SCHEDULE
Full Time. 24 hours in Plainville, CT and 16 hours floating at other health center sites.
QUALIFICATIONS
- Licensed as an Advanced Practice Registered Nurse (APRN) or Physician
- Board Certified as a Family Nurse Practitioner or Family Practice Physician
- Active DEA Controlled substance registration
- Must have at least one year of post-licensure primary care experience or 3 years of other post-licensure practice experience
ADDITIONAL POSITION REQUIREMENTS
Will be required to acquire and maintain certification in Basic Life Support for Healthcare professionals no later than 90 days from date of hire.ESSENTIAL DUTIES AND RESPONSIBILITIES
Conducts medical evaluations and formulates accurate diagnoses in accordance with the International Classification of Diseases.Manages prescribed medication, performs physical health examinations, conducts preventive screening and care.Develops and presents clear clinical impressions and recommendations that are specific and attainable.Prescribes therapeutic interventions appropriate to identified problems.Administers drugs and biologicals as required by the Clinic.Demonstrates knowledge of physical health concerns and effective medical interventions.Triages clinical emergencies and implements safety and consultation protocols.Utilizes evidence-based and best medical practice.Monitors patients' self-care management and self-care skills and provides prevention, wellness, and pertinent medical education.Provides requisitions for clinical studies, diagnostic tests, and laboratory studies.Evaluates and interprets results to identify and assess patient's clinical problems and health care needs.Demonstrates knowledge of internal and external levels of care, referral processes and resources.Makes clinically appropriate referrals for consultation with specialists, subspecialist, and allied health professionals as indicated.Ensures all patients’ medical records are completed timely and in accordance with organization policy.Works with care coordination team in designing, monitoring and implementing care plans.Maintains consistent communication with community providers, local and state agencies, and other allied professionals and services mutually involved in client care.Represents the Clinic in external and internal committee meetings as assigned.Communicates in a professional, positive, effective, and courteous manner with client, family, and all internal and external providers.Meets direct service productivity goals as defined by program.Attends and is prepared for scheduled meetings with administrative supervisor.Attends and is prepared for scheduled clinical meetings and consultation with supervising physician.Provides service in the office and / or community settings outside of the office (e.g., homes, schools, hospitals, institutions, other community locations) as defined by the program.Must be able to pass a respirator fit test at time of hire and on an annual basis.Providers will treat HIV, Hepatitis C, and provide gender affirming care within their primary care practice.Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served.Wheeler Clinic is an equal opportunity employer.
