The Case Manager is responsible for the delivery of services to residents and families; acting as liaison between treatment team, referral source, community agencies, court officials, outside therapists; maintains the integrity of the medical record of assigned residents; leads specific treatment team meetings; prepares and participates in inquiry system by taking inquiry calls and assists in disposition of calls under the guidance of Clinical Director. Provides assistance with resident care as needed.
Qualifications and Skills
Bachelor's Degree in Social Work, Counseling, Clinical Psychology, or related field, or 3 years of experience in Social Services field required.
Management experience with knowledge of psychiatric patient care techniques and an understanding of mental illness, preferred.
Must maintain a valid driver's license required.
May be required to work overtime and flexible hours.
