The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization dedicated to advancing the health and well-being of all New Yorkers. In partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives to advance community health throughout the city.
PROGRAM OVERVIEW
The Bureau of Health Equity Capacity Building (BHECB) seeks to implement collective action strategies to build community power and capacity to address health inequities and the social determinants of health. BHECB develops strategies, activities, and processes that build, strengthen, and maintain the skills and resources necessary to improve health outcomes in NYC's underserved communities.
Advancing Care and Equity for Diabetes Prevention and Management, referred to as “ACED”, is a DOHMH initiative aimed at decreasing the risk for type 2 diabetes and advancing health equity by addressing the social determinants of health that impact priority populations.
POSITION OVERVIEW
This role will report to the Executive Director, Community Resilience and Response Unit, Health Equity Capacity Building. The incumbent will coordinate across the ACED grant team and HECB to align ACED's diabetes prevention / management strategies with HECB's community engagement and chronic disease prevention goals.
The position is federally-grant-funded, and continued support for this role is contingent upon future funding.
RESPONSIBILITIES
- Support the Bureau with managing programs and deliverables to align and meet ACED goals and objectives.
- Liaise with project leads and other staff overseeing discrete projects in the Bureau’s portfolio.
- Develop, implement, and maintain internal project management workflows for HECB ACED.
- Coordinate across the ACED grant team and HECB to align strategies with community engagement and chronic disease prevention goals.
- Directly support liaising with Healthy NYC leadership teams.
- Catalogue and disseminate chronic disease-related resources.
- Develop written documents for cross-grant reporting and presentations.
- Other tasks as required.
