The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization dedicated to advancing the health and well-being of all New Yorkers. In partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives to advance community health throughout the city.
PROGRAM OVERVIEW
The Bureau of Health Equity Capacity Building (BHECB) seeks to implement collective action strategies to build community power and capacity to address health inequities and the social determinants of health. BHECB develops strategies, activities, and processes that strengthen the skills and resources necessary to improve health outcomes in NYC's underserved communities.
Advancing Care and Equity for Diabetes Prevention and Management, referred to as "ACED", is a DOHMH initiative within high-needs counties to decrease risk for type 2 diabetes through improved screening and increase access to community change programs. Using a collective impact approach, DOHMH will partner with community-based and clinical organizations to expand access to sustainable services to prevent diabetes and improve management.
POSITION OVERVIEW
This role will report to the Executive Director, Community Resilience and Response Unit, Health Equity Capacity Building. The incumbent will coordinate across the ACED grant team and HECB to align strategies with community engagement and chronic disease prevention goals.
The position is federally-grant-funded, and continued support for this role is contingent upon future funding.
RESPONSIBILITIES
- Support the Bureau with managing programs and deliverables to align ACED goals with Healthy NYC objectives, focusing on diabetes and chronic disease.
- Liaise with project leads and staff overseeing projects in the Bureau’s portfolio.
- Develop and maintain internal project management workflows for HECB ACED and associated strategies.
- Coordinate with the ACED grant team and HECB to align diabetes prevention strategies with community engagement goals.
- Support liaising with Healthy NYC leadership teams, attending workgroups and coalition meetings.
- Catalogue and disseminate chronic disease-related resources to community partners.
- Develop written documents for cross-grant reporting and presentations to stakeholders.
- Other tasks as required.
