Trilogy Health Services, an innovative senior living company is looking for a Master Social Worker to join the Dialysis team at our health campus. The goal of our Master Social Worker is to promote social functioning, psychological adjustment, and overall well-being in each renal patient.
Here are a few of the responsibilities of the Master Social Worker :
- Assessing the patient's financial, medical, social, and familial needs.
- Meeting the patient's needs through direct supportive counseling or through referrals to other agencies.
- Recommending changes in treatment based on the patient's current psychosocial needs.
- Providing case work and group work services to patients and their families in dealing with the special problems associated with end stage renal disease.
- As a member of a Home Dialysis Training Unit, participate in periodic home visits
