Dialysis Social Worker (MSW)

Trilogy Health Services, an innovative senior living company is looking for a Master Social Worker to join the Dialysis team at our health campus.  The goal of our  Master Social Worker is to promote social functioning, psychological adjustment, and overall well-being in each renal patient. 

Here are a few of the responsibilities of the Master Social Worker:

• Assessing the patient’s financial, medical, social, and familial needs.
• Meeting the patient’s needs through direct supportive counseling or through referrals to other agencies.
• Recommending changes in treatment based on the patient’s current psychosocial needs.
• Providing case work and group work services to patients and their families in dealing with the special problems associated with end stage renal disease.
• As a member of a Home Dialysis Training Unit, participate in periodic home visits

• Holds a master’s degree in social work with a specialization in clinical practice from a school of social work accredited by the Council on Social
• Has served at least two years as a social worker, one year of which was in a dialysis unit or transplantation program prior to September 1, 1976, and has established a consultative relationship with a social worker who qualifies under § 494.140 (d)(1
• The social worker must meet the licensure requirements in the state of practice.
• Worked at least two years as a social worker, one year of which was in a dialysis facility or transplantation program.
