WHAT YOU WILL DO
Identify and validate care gaps in provider appointments.
Enter gap in care in upcoming appointment note to provider.
Follow-up review after appointment to confirm gaps in care closed during visit.
Assist with closing quality gaps in care.
Review gaps in care report and validate care gaps by using health plan quality reports, Epic reports and health plan systems (i.e. Availity, Health BI).
Monitor gap closure by utilizing an excel work list.
Assist with year-end and annual quality audit.
Assist with coordinating PCP visits.
Outreach to members/beneficiaries to close gaps in care.
Mail educational information to members/beneficiaries regarding preventive care quality metrics.
Educate members/beneficiaries regarding preventive care and document as an encounter in Epic.
Assist with other focused quality initiatives.
WHAT IT TAKES TO SUCCEED
Work autonomously and be accountable in a complex environment.
Adapt within an ever-changing healthcare environment.
Strong communication skills.
Medicare and commercial insurance knowledge.
Knowledge of electronic health records.
Qualifications
- Required High School Diploma/GED
- 1 year Clinical Experience Preferred
- 1 year Medical Assistant Preferred
