Position Title: Case Manager
Reporting Relationship: Manager of Program Operations
I. Position Summary:
The Case Manager works one-on-one with clients in shelter to complete needs assessments, establish goals, and formulate service plans while keeping abreast of domestic violence and sexual assault issues. This associate links clients with community resources as necessary and develops and maintains working relationships with referral agencies. This associate will assist in transporting clients in the agency van. The Case Manager plans and facilitates weekly support groups.
II. Specific Task/Duties
1. Works with clients individually in shelter on a weekly basis to discuss their goals and progress.
2. Links clients to community services as necessary, maintains working relationships with those agencies including phone contact and outreach and maintains client resource packet.
3. Maintains documentation for client files including service plans, session notes, matrix scores, and daily statistics.
14. Meets with all clients within 72 hours of intake to establish goals and assess need for services.
5. Attends and participates in all required staff meetings and trainings including weekly client reviews.
6. Assists as needed for client assistance and/or crisis intervention.
7. Assists in transporting clients in the agency van as necessary.
8. Other duties as assigned.

IV. Position Requirements
Minimum Preferred Education:Bachelor’s degree in psychology, social work, or related field.
Applicable Experience: Experience in social services working with diverse populations. Prior case management experience. Experience working with victims of domestic violence and/or sexual assault.
