Position - Case Manager
Regional Health Systems is Northwest Indiana’s largest provider of community-based mental health care, primary health care and addiction treatment services. We offer adults and children—both uninsured and insured—integrated care through our community mental health center and federally qualified health center. Regional Health Systems is a member of the Regional Care Group.
Position Description:
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a critical role in supporting individuals and families by providing guidance, resources, and advocacy. This position requires a strong understanding of behavioral health, child welfare, and crisis management to effectively assist clients in navigating their challenges and achieving their goals.
Essential Functions:
- Conduct comprehensive assessments to identify client needs and develop personalized service plans.
- Provide ongoing support and case management services to clients, ensuring they have access to necessary resources.
- Collaborate with multidisciplinary teams, including healthcare providers, social workers, and community organizations.
- Facilitate communication between clients and service providers to ensure continuity of care.
- Monitor client progress and adjust service plans as needed to promote positive outcomes.
- Utilize motivational interviewing techniques to empower clients in their decision-making processes.
- Maintain accurate documentation of client interactions, progress notes, and case files in compliance with organizational policies.
- Stay informed about community resources, including Section 8 housing assistance and workforce development programs.
Requirements:
- Bachelor's Degree in Psychology, Sociology or related field
- Valid driver's license and clean driving record
- 1 year of experience

Job Type: Full-time
Pay: $17.51 - $22.76 per hour
Expected hours: 40 per week
