What are the responsibilities and job description for the Senior Nurse Coordinator position at Franciscan Ministries?
About the Role:
The Senior Nurse Coordinator will play a vital role in planning, organizing, and directing the Minimum Data Set (MDS) process in accordance with current applicable federal, state, and facility standards, guidelines, and regulations.
This position is responsible for ensuring accurate completion of the MDS tool to facilitate appropriate reimbursement.
Key Responsibilities:
- Conduct and coordinate the development and accurate completion of the resident assessment (MDS, CAAS, and Care Plans), in accordance with current rules, regulations, and guidelines governing the facility.
- Complete timely submissions of the MDS and monitor all related reports, including but not limited to Validation reports, Case mix reports, 5-Star Report, Quality Measures, HMX/Vision reports, PointRight, etc.
- Communicate relevant information to the Interdisciplinary Team/Management Team and participate in QAA process as necessary.
- Ensure resident trackers, discharges, and MDSs are opened/closed appropriately, ARDs, and Completion dates are set and entered on the Resident Care Conference Schedule.
- Responsible for communicating all necessary MDS details to the Interdisciplinary Team in a timely manner to ensure accurate completion of the MDS process.
- Verify accurate completion of resident face sheets, including ranking diagnoses and coding on admission and throughout the MDS process.
- Ensure accurate and timely completion of assessment and forms related to the MDS, including but not limited to Braden, Skin Grids, Admission, Falls, Restraints, Side rails, AIMS, Bowel, and Bladder, etc.
Requirements:
- Associate's or Bachelor's degree in Nursing
- Active LPN or RN license that permits practice in the state where hired
- 1-2 years of experience in MDS
- Able to demonstrate compassion when working with residents
