The successful candidate will be an RN who advocates and promotes the facility's services to hospitals, case workers, discharge planners, physician offices, and other clinical providers. Key responsibilities include:
Key Responsibilities:
- Following up on hospital referrals
- Evaluating residents for proper placement
- Explaining the facility's services
- Maintaining and developing referral sources
Requirements:
- Valid Wisconsin RN License
- Two years' experience in assisted living, senior housing, or senior healthcare
