In addition to helping individuals develop acceptable home plans, this position will provide linkages to community resources to assist the individual’s stable reentry into the community. A. EDUCATION/TRAINING

Bachelor’s Degree in Human Services/Social Services or related field; certification under the Alliance of Information and Referral Systems (AIRS) is preferred but not required.

B. WORK EXPERIENCE

A minimum of four (4) years human service or behavioral health experience and knowledge of human services in Franklin County, and computer experience in word processing and database required. Or, any equivalent combination of experience and training.

Community Connection Forensic Housing Case Manager January 3, 2023

Position Description Non‑Exempt

OVERALL OBJECTIVE OF JOB

This position is responsible for working with the forensic population in the Franklin County Jail and on supervision with Franklin County Adult Probation who are referred due to housing needs. In addition to helping individuals develop acceptable home plans, this position will provide linkages to community resources to assist the individual’s stable reentry into the community. This position will act as the central point of contact for services providers and agencies including adult probation.

ESSENTIAL FUNCTIONS OF JOB

  • Assess housing barriers of referred individuals to determine housing and service needs.
  • Complete necessary paperwork, reports, data entry, and lease coordination to assist with housing stabilization.
  • Completion and compile paperwork, such as leases, Housing Needs Assessment, Housing Goal Plans, releases, tenant agreements, and income documentation.
  • Provide education to program participants on topics such as budgeting, cleanliness, leases, landlord expectations, communication with landlord, and use of community resources for assistance.
  • Assist in the development of and encourage adherence to a personal budget through housing and budget counseling sessions; provide budget counseling and education to assist consumers in maintaining their housing.
  • Monitor program participants who are receiving financial assistance.
  • Conduct follow-up visits with individuals in their housing units.
  • Work with program participants to resolve barriers to housing.
  • Compile data for reports as required by the program.
  • Travel to various sites, including the Franklin County Jail, within the region to conduct intakes and complete other transactions relating to program services.
  • Provide mediation and advocacy with landlords on the program participant’s behalf to develop a workable plan to obtain and maintain housing.
  • Assist program participants in locating and securing safe, affordable housing of their choice.
  • Apply knowledge of residential lease contracts to educate consumers of their rights and responsibilities.
  • Actively engage current and potential landlords to improve the availability of affordable housing options.
  • Participate as part of the program treatment team for the purpose of ongoing program monitoring and community engagement.
  • Schedule, accompany and transport program participants to meetings and/or appointment with service providers as needed.
  • Facilitate and attend meetings with program participants, providers, others to ensure appropriate coordination of services.
  • Advocate for program participants with the court system, assistance office, and/or other areas when appropriate and as needed.
  • Respond in a timely manner to program participants who are in crisis and work with them to obtain services and assistance needed for stabilization.
  • Obtain needed documentation from program participants as needed to obtain services.
  • Maintain ongoing contact with program participants, landlords and other service providers as needed to ensure continued participant self-sufficiency and the appropriate coordination of services and support systems.
  • Document all activities and create files on each program participant who receives services.
  • Develop resources within the community to meet the needs of program participants before they reach crisis level or before problems become unmanageable.

OTHER DUTIES OF JOB

  • Attends training, meetings and in services, as required.
  • Performs other duties as assigned.

SUPERVISION RECEIVED

Receives occasional instruction and limited supervision in regard to daily work duties.

SUPERVISION GIVEN

Receive direction and supervision from Community Connection Housing Programs Specialist.

WORKING CONDITIONS

  • Works indoors in adequate work space, temperatures, ventilation and lighting.
  • Works with average indoor exposure to noise and stress, but subject to frequent disruptions.
  • Normal indoor exposure to dust/dirt.
  • Travels occasionally, as needed, to perform essential functions of job.
  • Willing to travel in all the counties we serve, and other counties as required.

PHYSICAL/MENTAL CONDITIONS

  • Must possess ability to record, covey and present information, explain procedures and follow instructions.
  • Must be able to sit for long periods throughout the workday, with intermittent periods of standing, walking, bending, twisting and reaching, as necessary, to carry out job duties.
  • Dexterity requirements range from simple to coordinated movements of fingers/hands; feet/legs; torso, as necessary, to carry out job duties.
  • Sedentary work, with occasional lifting/carrying of objects with a maximum weight of thirty pounds.
  • Must be able to pay close attention to details and concentrate on work.

QUALIFICATIONS

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED

  • Must possess knowledge of referral agencies and the ability to interact with agencies on behalf of clients’ needs.
  • Must be able to fluently speak, write and understand the English language in an understandable manner in order to carry out the essential functions of the job.
  • Must possess good communication and interpersonal skills.
  • Must possess initiative and problem solving skills.
  • Must possess the ability to function independently, have flexibility and the ability to work effectively with diverse clients, co‑workers and others.
  • Must possess the ability to maintain confidentiality in regard to client information and records.
  • Must possess the technical knowledge to proficiently operate a personal computer, including word processing, database and Internet, and be able to operate other office equipment, as needed.
  • Must be able to prepare presentations and do public speaking.
  • Must possess a valid driver’s license and a willingness to travel as needed to carry out essential job duties.
  • Must possess knowledge of principles and practices of social work and the ability to apply same to essential job duties.
  • Must possess ability maintain accurate records and reports in accordance with established policies and procedures.
  • Ability to build rapport with diverse consumers, coworkers and colleagues through effective communication, listening skills and empathy.
  • Ability to adapt to a variety of situations and individuals, ability to work independently and also work on team projects.
  • Ability to develop and maintain an understanding of eligibility criteria and policies related to various federal and local public benefit programs.
  • Ability to maintain confidentiality regarding all agency business, employee and consumer matters.
