Essential Functions:
  • Perform accurate and timely Patient Registration in the Salesforce EMR utilizing information present throughout multiple systems
  • Assist with in-hospital recruiting as needed:
    • Establish initial contact with families at local birthing hospitals. Successfully engage, recruit, and schedule families for nurse home visits.
    • Build relationships with hospital birth center management and staff including attending meetings, providing educational in-services, and troubleshooting recruitment problems.
  • Schedule clients by phone that are missed during in-hospital recruiting.
  • Conduct post-visit connection (PVC) phone calls with families, track services contacted and received, and provide re-referrals when necessary.
  • Act as primary public contact to answer questions and reschedule home visits as needed. Follow up with families in a timely manner, show good customer service and coordinate coverage in this area when not in the office.
  • Work as an active member of the Family Connects team by attending all regular staff meetings, Family Connects planning meetings, in-service trainings, and other meetings as required.
  • Complete other duties and tasks as assigned by Clinical Manager and program leadership.

Minimum Education, Training, and Experience Required:

  • Bachelor’s degree in a related field preferred but not required.
  • Experience providing administrative support to a clinical program strongly preferred.
  • Demonstrates ability for collaborative work in a team environment, as well as ability to work independently to meet expected deadlines.
  • Demonstrates excellent verbal and written communication skills with strong attention to detail and multitasking skills.

Required Knowledge, Skills and Abilities:

  • Ability to communicate clearly and professionally with a range of community stakeholders.
  • Ability to competently use personal computer and printer, word processing, spread sheet and database software applications
  • Ability to work independently, as well as in teams.
  • Ability to work well with diverse populations through inclusive and culturally responsive practices.

Physical Requirements:

Medium Work – Lifting up to 50 pounds on an infrequent basis (less than one lift every three minutes) and/or carrying up to 25 pounds while walking. Pushing or pulling up to 75 pounds infrequently or up to 115 pounds occasionally (no more than once per hour).

Awkward Posture: Posture is the position your body is in and its effect on the muscle groups that are involved in the physical activity. Awkward postures include repeated or prolonged reaching, twisting, bending, kneeling, squatting, working overhead with your hands or arms, or holding fixed positions.
