Job Duties:
- Assumes responsibility for management of the discharge plan for the patients stay throughout the program.
- Contacts referral sources and family members in order to gather clinical information for the multidisciplinary team and reports to the treatment team.
- Interacts with members of the medical/clinical team to provide a flow of communication and accurately documents in the medical record the findings and data that supports level and intensity of service rendered.
- Functions as a member of the multidisciplinary team and assist in facilitating the treatment team process.
- Works closely with outside agencies to place patients after treatment to manage continuity of care.
- Facilitates communication to outside agencies to ensure that these patient rights, care, and needs are met.
PREFERRED KNOWLEDGE, SKILLS, AND/OR EXPERIENCE REQUIRED:
Education: Bachelor's Degree - preferably in a Social Services field
License: N/A
Experience: 1 years case worker and/or discharge planning activities
