**Job Overview:**
The Director of Nursing/Director of Services (DOR/DOS) will oversee and manage the nursing division of Freedom Home Health to ensure smooth day to day operations, compliance with company, state, and federal guidelines, and to provide guidance as needed to administrative and support staff. The role of the DOR/DOS may also include providing nursing care directly to patients during home health visits (which may include Oasis/Start of Care services, routine visits, discharge planning, etc.), providing training/onboarding to new staff, and overseeing the management of medical information/records to ensure compliance standards.
**Duties:**
- Ensuring the care offered by Agency is consistently available to all patients.
- Coordinating and integrating care provided by Agency staff, including contractual providers.
- Collaborating on development and implementation of policies and procedures which guide and support the provision of care.
- Recommending resources needed to provide care.
- Collaborating with Agency leadership and appropriate staff to develop the organizational plan as it pertains to patient care.
- Participating in activities relevant to professional services furnished including the development of qualifications and assignment of agency personnel.
- Participating in the development and implementation of the Performance Improvement Plan.
- Being available (or designee) at all times in person or via telecommunications.
- Assures that a client's Plan of Care is executed as written.
- Oversees infection control and quality improvement processes.
- Assures a reassessment of a client's needs is performed by the appropriate healthcare professional a) When there is a significant health status change in the patient's condition; b) At the physician's request; c) After hospital discharge.
- Overall responsibility for supervision of nursing care and nursing personnel
- Interviewing and selection of Professionals and non-Professionals nursing staff
- Plans and organizes an effective on-going in-service program, and makes known to staff educational opportunities as found in the community
- See that all State and Federal rules and regulations, as they pertain to the nursing department, are carried out so that no deficiencies are found in inspections.
- Provide for timely, thorough, and accurate clinical documentation in accordance to HIPAA requirements and Freedom Home Health Policies and Procedures standards regarding patient confidentiality.
- Complete all required documentation on their due dates.
- Identify the patient or family/caregiver needs for additional services and refer to other healthcare professionals as appropriate.
- Participate in any required case conferences, skills competency sessions, in-service presentations, or other such activities that may be deemed mandatory by your supervisor.
- Provide for discharge planning including any further recommendations for equipment or devices, further nursing services, supervision levels to ensure patient safety, family education/training to facilitate safe discharge from home health services.
- Perform your duties in a safe, professional, and ethical manner.
- Assist with scheduling patient care sessions to ensure full compliance with established frequency of services.
**Qualifications:**
- Bachelor of Nursing
- Nursing license in Arizona that is in good standing
- Minimum 2 years work experience as a Registered Nurse in the home health field.
- Prior Director of Nursing or Director of Services preferred.
- Current CPR Certification
- Valid Driver’s License
- Proof of liability insurance on vehicle
- Current TB clearance preferred
- Fingerprint Clearance Card
