Job Summary
The social worker provides social work services to patients and their families in order to enhance psychosocial functioning and continuity of care. The social worker conducts psychosocial assessments of patients/families. The social worker, in collaboration with the multidisciplinary team, develops and implements discharge planning for identified patients. The social worker provides psychosocial intervention with trauma victims, patients with psychosocial disorders, and patients having a difficult time adjusting to an illness.
Job Responsibilities
- Screen and prioritize all referrals.
- Complete psychosocial evaluation to identify patient's physical, financial, and psychological needs. Provide short-term counseling for patient/family as needed, including bereavement.
- Optimize patient outcomes by referring patients/families to appropriate community resources.
- Evaluate and discuss needs and options for discharge, and complete respite admission papers with patient/family when appropriate.
- Collaborate with medical and nursing staff, other health care professionals and community agencies to assure the integration of social work services in patient care.
- Communicate information to health case manager regarding patient's needs that may delay service or impact the length of stay.
- Organize, lead, and participate in patient care planning conference as needed.
- Assist patients with completion of advance directives.
- Document social work intervention in medical record.
- Provide on-call services.
- Display competency for the Medical Social Worker role as evidenced in the annual performance appraisal following the FHN PRIDE Standards.
- Other duties may be assigned.
Qualifications
Requirements
Education: Bachelor’s Degree in Social Work, Sociology, or Psychology
Required Experience: Experience in healthcare setting
Special Skills and Abilities:
- Requires the ability to write information and to enter/retrieve information from a computer.
- Requires the ability to speak clearly and to hear conversations when there is background noise.
- Requires the use of universal precautions due to exposure to blood-borne pathogens.
- Work involved mobility throughout the hospital.
