Description
Assist clients with social service requests at the front desk, triage/reception area or through the intake department; greets clients in a timely manner and directs clients to other service locations within the building; make referrals to outside community resources and assists in the screening and coordination of special needs clients.
Essential Job Functions
- Conduct initial interview and input essential information into a database.
- Assess client needs in a timely, courteous, and efficient manner to determine the extent of required needs and/or services.
- Determine client eligibility for agency supplied goods (e.g. bus tokens, vouchers, food, etc.) and follow protocol for distribution; make additional referrals and provide basic client information to community resources.
- Complete the appropriate internal agency referrals, including housing, crisis intervention assessment, employment and public benefit support services.
- May also help to coordinate services, scheduling and referrals for Back on Track to include other community providers.
- Coordinate referrals as needed for mental health status, medical conditions and other issues outside the scope of intake.
Qualifications
Minimum Education/Experience
- Associate or Bachelor’s Degree (degree in social work strongly preferred) plus one year of related experience in Social/Human Services, or three years of directly related experience.
- Experience providing intake and basic clientele assessments.
- Basic knowledge of community resources is required.
- Fluent in Spanish, French, or Fulani (both written and oral) preferred, but not required.

- Excellent inter-personal skills requiring good judgment and tact to communicate effectively with clients demonstrating a range of emotional behavior.
- Ability to assess client needs and provide direction and resources.
- Respect for diversity in clientele and staff.
- Ability to exercise sound judgment and problem solve a variety of situations.
- Knowledge of inhouse and external community resources.
- Ability to work in a fluid and occasionally high demand environment.
- Working knowledge of Microsoft Outlook, Microsoft Word, and Microsoft Excel.
- Ability to accurately enter information into a database is required.
